If your child requires medication at school, you must also provide signed copies of the Prescription Medication Form or the Non-Prescription Medication Form.
We must also have the signed Indemnity Agreement on file. See below.
Please Note: Registration packets must be complete in order to reserve your child’s spot in the preschool. All paperwork must be completely filled out and a current shot record must be attached to the paperwork along with the non-refundable registration and supply fees.
